You can complete a template by following these steps:

This Assessment will be a "dummy" Assessment because it will serve as a template. The sections and fields you need to focus on are bolded below:

Upload a Previous Report

Enter Project Details
1. Include the words "Master Template" in the name of your Assessment. This will make it easier for users to find this template when they insert it into their Assessment.
2. Click SAVE & CONTINUE
Select the Service Area
1. Include the broadest Service Area that applies to your group
2. Click SAVE & CONTINUE
Select the Benchmarks
- This step will not impact your template. Click SAVE & CONTINUE
Select Priority Populations
- This step will not impact your template. Click SAVE & CONTINUE
Select Health Themes
1. Include the Health Themes exactly as you want them to appear in your template
2. Click SAVE & CONTINUE
Add Topics
1. Add the Topics that you want to appear in your template
2. Click SAVE & CONTINUE
Customize Your Insights
- This step will not impact your template. Click SAVE & CONTINUE
Data Collection Methods
- This step will not impact your template. Click SAVE & CONTINUE

Once you have completed and saved all steps, your template will be complete!
